The "three firsts" should also be carefully examined: the first sentence, the first page, and the first chapter.  The first sentence must hook the reader to continue reading the first page, which in turn pulls the reader through the first chapter.  If the reader isn't hooked by the end of the first chapter, then you've likely lost him or her for the rest of the book.
